請填妥以下表格(請以正楷填寫)，並將表格列印，連同存款收據，傳真至8344 5172。之後我們會給你確認電郵，在收到會員證前，確認電郵會是你的臨時會員證。會員證會於印妥後以掛號方式郵寄給你。

Please complete in block letters the following application form. Afterwards, print it out and fax it to 8344 5172 along with the deposit receipt. Before the receipt of an official membership card, the confirmation email you receive will serve as your temporary membership card. The official membership card, once being issued, will be posted to you by registered mail.

** You are welcomed to apply to become a member at any time. Note that the membership cards will only be issued twice a year depending on the circumstances; therefore after your application is received, it may take several months before the card is mailed to you.
